Industrial safety

Two worker deaths and one serious injury at Endeavor Mine

28 October 2025–at least 27 February 2026 · documented · high materiality

On 28 October 2025, an explosive device unintentionally detonated underground at Endeavor Mine; two workers died and a third was seriously injured. The NSW Resources Regulator opened an investigation. Polymetals’ February 2026 half-year report said internal and external reviews were continuing, that the outcome and any liability were uncertain, and that no action had then been taken against or notified to the company. No reviewed source attributes personal causation to this executive.

Attribution: Employer context; no individual link established. The incident occurred at an operation owned by the employer. Available sources establish no individual causal role; team-level oversight cannot be equated with personal fault.

Relevance: Material safety and operational oversight issue for current mine leadership. The investigation remained unresolved in the latest reviewed company filing.

Endeavor Mine fatality investigation information release IIR25-08 (Incident date 28 October 2025; investigation status in November 2025 release); Half-year report for period ended 31 December 2025 (Directors’ report, p.10, incident update and status as at 27 February 2026)

Governance and interests

Inadvertent Listing Rule 10.14 breach involving loan-funded shares

December 2024 issue; breach disclosed 22 December 2025; cure completed March 2026 · issuer reported · medium materiality

Polymetals disclosed that 250,000 shares were issued to a nominee company controlled by him. The company stated in December 2025 that it inadvertently failed to obtain the separate shareholder approval required by Listing Rule 10.14, despite general Share Plan approval, and self-reported the breach to ASX. Shareholders approved a selective buy-back at the 20 February 2026 EGM; an Appendix 3H records cancellation of all 562,500 affected shares on 6 March 2026, with consideration offset against repayment of the loan. The issuer called the lapse administrative and inadvertent. No deliberate personal conduct or further sanction is established in the reviewed sources. This is a documented governance control failure during the directors’ tenure, followed by an issuer-reported cure.

Attribution: Role overlaps event; causation unproven. The company attributed the approval failure to an administrative oversight and self-reported it. The sources identify the directors who received the shares but do not attribute the approval failure to either person individually.

Relevance: Director equity awards and compliance with shareholder-approval rules are directly relevant governance matters.

Inadvertent Breach of Listing Rule 10.14; Selective Buy-Back (ASX release, 22 December 2025: issue amounts, approval lapse, self-report and proposed remedy); Appendix 3H: Notification of cessation of securities (Appendix 3H, 10 March 2026: approval on 20 February and cancellation on 6 March)

Relevance: Role match is relevant to present oversight, but the source set is too thin to assess delivery or effectiveness.
